Pre-visit intake forms

Once you have booked an appointment you should get an email from my charting software, called Milknotes. If you don’t see an email within 24 hours please check your spam folder and email me. At this time, the intake form is not customizable by me and I feel it can be a little overwhelming. Please do your best to complete as much as you can so that I can spend our appointment time assisting you instead of doing paperwork.

What to expect with an initial postpartum home visit consultation

My goal is to support you and your feeding goals. Please don’t worry about cleaning your home before I arrive—I’m there for you, not your house. Wear whatever feels comfortable (I know I will!). If possible, try to have your baby ready to feed (but not hangry) so we can make the most of our time together. When you book the appointment you should receive an intake form to fill out, please be sure to complete this before our visit.

I do ask that pets be closed off from the space where we’ll be working, just so I can focus fully on you and your baby. When I arrive, we’ll briefly review intake paperwork, talk through your birth history and any concerns, and get a weight on your baby before starting a feeding. I will bring a scale with me. I’ll observe latch and positioning, learn more about your goals and challenges, and then we’ll work together to create a care plan that feels right for you.

After your appointment I will send you a copy of the care plan that we discussed. Initial lactation visit also includes a physician report faxed to your pediatrician and/or OBGYN. Forgot something? One week of follow-up included for quick questions, clarification, or just to check in.
